When purchasing the course , I understand that yoga classes may be physically strenuous for some, and I voluntarily allow my child/ward to participate in them, with full knowledge that there is a small risk of personal injury. By signing this form I confirm that I accept that no responsibility for accidents or injuries or loss/damage to personal property rests with the supervisory staff unless proven to be caused by their negligence. I certify that the above details are true to the best of my knowledge and if any information changes you will notify Melinda Szepesi immediately.
